Volunteer Background Check - Release Form

Weekend Survival Kit

It is our goal at Bath Community Schools to have every student succeed to their fullest academic potential. We realize that there are often factors that are out of our control that interfere with children's ability to learn, but controlling all the factors we can is our focus. One of those factors, simply is hunger (read more in this letter home to parents).

Medication Forms

If your child needs to have medication dispensed by the school, the prescribing doctor is required to complete one of our medication forms.  The medication will need to be dropped off in the office to a staff member by a parent/guardian with the completed form.  There are 2 separate forms, one is for epi-pens and inhalers, and the other is for any other medications.  Thank you.
